Our work is guided by our CARE PLAN: The CARE PLAN guides us throughout our work and is frequently updated as we monitor the services we put in place. If needed, we make REFERRALS to Attornies, Financial Planners, Adult Day Centers, or Rehabilitation Centers. We can ACCOMPANY TO MEDICAL APPOINTMENTS, and serve as the LIASON between the doctor/patient/family accurately reporting the situation and follow-up care to your family after each visit.
At ElderOptions our client might be an older adult ready to engage and enact a plan to address their own needs, or the family members or loved ones of an older adult, who seek consultation and guidance to access the best services for someone in need of care. ElderOptions prides itself on serving a diverse range of family constructs, and serving clients in Chicago, New York City and other geographical locations.

If you live far from your older loved one, we’ll COMMUNICATE with you with the frequency you desire and provide updates on care concerns. We make HOME VISITS on a regular basis, and are available to check-in, in case your are worried, and ACCOMPANY your loved one to a hospital or to urgent care if needed. We can ADVOCATE for your older loved one in a medical or financial situation. We offer ADVICE about long and short-term planning needs and Geriatric Care Management Services – Resource Referrals to Attorney’s, Financial Managers 24 hour 7 day/week service, Plan of Care for Short or Long-term needs, Referrals to Adult Day Centers, Rehab Therapies, medical alert systems Liaison with medical professionals – accompanying client to Dr. Appointments and communicating with families if needed.
